Patient history

A 58-year-old male with known history of fistulas presented at Holy Family Hospital, New Delhi, India. The patient underwent surgery for fistula 5 months back and now on follow up shows recurrence of fluid discharge. He underwent a MR fistulogram on Multiva 1.5T. 

MR examination

The MR examination was performed on a release 5.1 Multiva 1.5T system using the HeadSpineTorso (HST) coil.

The imaging suggests a fistula with a partially fibrosed tract in the left posterior perineal tissue extending superiorly into irregular thick walled collection in the peri-anal fossa. Another fistulous tract extending from it into a small septated thick walled collection in the right peri-anal region. There is also a secondary fistulous tract extending from this, that has opening into anal canal.

Impact of imaging with Multiva 1.5T

MR imaging of Fistulogram requires very high quality imaging to depict small fistulas, their pathway, openings and characteristics. In this case-study, non contrast MR Imaging on a Multiva of the perineal region in small FOV sagittal, coronal and axial planes is able to demonstrate fistula . Excellent Multiva performance is shown in imaging the details of fistulogram.
