Improved quality of cine B-FFE images with Ingenia 3.0T has a diagnostic impact

Patient history

55-year-old male with suspected coronary artery disease, diabetes and arterial hypertension who underwent a vasodilator stress CMR as a follow-up exam. A prior CMR exam on a 3.0T system without MultiTransmit had demonstrated both normal morphology and function of the left ventricle.
Ingenia 3.0T with Anterior and integrated Posterior coils was used for a typical cardiac exam. Spatial resolution in cine B-FFE was 1.5 mm. Results are compared with an earlier exam on our Achieva 3.0T without MultiTransmit. Spatial resolution on Achieva 3.0T was 2.0 mm. Breath hold time was about 8-10 seconds in both exams.

In the cine B-FFE images from Achieva 3.0T without MultiTransmit the contrast between blood and myocardium in the 4-chamber view is reduced, particularly in the right ventricle. Furthermore, there are more flow related artifacts, which can be appreciated better in the movies compared to the still frames.

The Ingenia 3.0T images have fewer artifacts, higher CNR and higher spatial resolution. In the 3-chamber view the presence of a myocardial crypt can be appreciated only on the Ingenia 3.0T images as a result of the much better overall image quality.

Impact of Ingenia 3.0T

Compared to the Achieva 3.0T images, Ingenia 3.0T images show a much improved image quality of cine B-FFE due to better shimming with fewer artifacts, higher CNR and higher spatial resolution. This is especially important for the exact determination of cardiac volumes and the assessment of regional wall motion and morphology. In this case it led to the new diagnosis of a myocardial crypt.
