Multi-parametric MRI and bone scan discordant lesion in prostate cancer patient

Patient history

After a bone scan a patient with known prostate cancer underwent multi-parametric MRI at University College London Hospital.

MR examination

A multi-parametric MRI exam was performed using Ingenia 3.0T with the dS Torso coil solution.

No iliac bone lesion is seen on the bone scan. The coronal mDIXON inphase image demonstrates subtle hypointense T1 signal within the left ilium (arrow). The axial diffusion b1000 image (inverted contrast) demonstrates restricted diffusion at the corresponding site within the left ilium (arrow). The axial T2-weighted image at the site confirms a typical appearance of a low T2 signal metastatic deposit (arrow).

Multi-parametric MRI demonstrates a lesion.

Clinical impact of multi-parametric MRI

Combining multiple contrast mechanisms provides more information than any one particular technique, which is powerful when not all lesions are seen by any one technique. In this case multi-parametric MRI demonstrates a lesion that is not seen on the bone scan, suggesting multi-parametric MRI may be more sensitive than bone scan.
