ExamCard purpose

This Release 2 SmartExam ExamCard is used for patients with a seizure disorder, especially where the suspected seizure focus is thought to be in the temporal lobes.

 

It is the ability to differentiate gray from white matter that helps to assess the presence of subtle hippocampal abnormalities and cortical malformation associated with temporal lobe epilepsy. This ExamCard incorporates a 3D TFE sequence that has been modified to increase grey/white matter differentiation.

Notes:

Scan 1 is a 59-second SmartSurvey.

Scan 2 is a standard reference scan.

Scan 3, DWI, is a standard brain sequence, used to create an ADC map.

Scan 4 is a quick sagittal localizer, used to plan the coronal series.

Scan 5, T2w AX FLAIR, covers the whole brain and is used to check for signal abnormalities in the cerebral hemispheres.

Scan 6-9 are aquired in an oblique/coronal plane perpendicular to the temporal lobes.

Scan 8 contains sub-millimeter slices to show subtle cortical abnormalities. The isotropic voxels enable 3D surface rendering and reformatting in different planes.

Scan 9, T1w COR IR CLEAR, produces Real images to show grey/white matter differentiation and anatomy.

Scan 10 is a routine post contrast sequence.

Patient preparation

The 8-channel SENSE Head coil was used.

Contrast agent / injection protocol

The standard gadolinium dose of 0.1 mmol/kg was used.
