This pediatric patient with a history of pneumonia with pneumatocele vs diaphragmatic hernia came to Phoenix Children's Hospital and was referred for a CT abdomen scan. The technologist used the IQon Spectral CT and Spectral CT Viewer to perform the exam.
The gallbladder is contracted with pericholecystic fluid. The images acquired on the IQon Spectral CT showed:
- Very large right-sided hydropneumothorax with complete collapse of the right lung. Small to moderate left pleural effusion. Nonspecific interstitial change in the left lung which could reflect edema, pulmonary venous congestion. Aspiration also possible. Mediastinal lymphadenopathy, likely reactive.
- Moderate amount of ascites throughout the abdomen.
- Heterogeneous appearance of the liver, likely secondary to phase of contrast. If there are concerns for liver pathology ultrasound with Doppler is recommended.
- Bilateral moderate hydronephrosis and hydroureter. Bilateral renal scarring.
The IQon Spectral CT's low MonoE capabilities improved the image quality of the anatomy surrounding the gall bladder.
