Pediatric brain - Multiple cranial and facial fractures

Clinical history

This 6 year-old male patient was an unrestrained backseat passenger involved in a motor vehicle accident. The patient was referred for a brain CT scan. The technologist used the IQon Spectral CT scanner and Spectral CT Viewer to perform the exam.

The brain CT images acquired on the IQon Spectral CT showed:
  1. Large right frontal lobe hematoma with scattered pneumocephalus and subarachnoid hemorrhage.
  2. Punctate focus of hemorrhage in the right genu of corpus callosum and likely within the atrium of the left lateral ventricle.
  3. Thin subdural hematoma along the falx.
  4. Extensive comminuted anterior cranial fossa, right greater than left frontal, right parietal and facial fractures as described above. Of note, non-displaced fractures through the sphenoid sinuses including the anterior wall of the left ICA canal. Consider CTA of the head to evaluate for vascular injury.

Clinical significance

In this case, shear injury not typically seen on CT was better visualized with IQon Spectral CT. The IQon Spectral CT provided enhanced visualization of a hemorrhage using MonoE results, as well as better visualization of corpus callosum.
