Clinical history

This 12 year-old female with refractory Evans syndrome presented at Phoenix Children's Hospital with abdominal pain, distention and diarrhea. She was admitted for bowel decompression and pain management. Given her history of recurrent diarrhea and refractory Evans syndrome, there was some concern that there was a different or additional underlying condition predisposing her to recurrent infection.

She was then referred for an abdomen CT scan. The technologist used the IQon Spectral CT scanner and Spectral CT Viewer to perform the exam.

The images acquired on the IQon Spectral CT showed splenomegaly. Circumferential gastric wall thickening mid to distal stomach and pylorus as above. Differential considerations include generalized gastritis/edema and/or inflammation, gastric lymphoma is considered, although there is no significant regional adenopathy, less likely gastrointestinal stromal tumor (GIST) secondary to relative low density of the gastric wall.

Clinical significance

The low MonoE images acquired on the IQon Spectral CT improved the visualization of the vascular structures of the abdomen, enabling a lower prescribed threshold of contrast dose for the pediatric patient.
