Advanced liver imaging on Ingenia 3.0T

These images demonstrate the capabilities of the dS Torso coil used on Ingenia 3.0T. This case study was done at Tokai, University Hospital, Japan.

MR examination

In this examination the patient entered the magnet feet first.
A breath hold mDIXON is used for reconstructing 4 image types: Water Only, In Phase, Out Phase, Fat only. In this example the breath hold time is only 11 seconds.

mDIXON produces four types of images:

For the dynamic liver scans, mDIXON is used with only the ‘Water only’ image type reconstructed.
Each dynamic phase has a breath hold of only 14 seconds.
The liver lesions are well demonstrated, and we can see the robust fat suppression that is achieved.

The protocol has 2 diffusion sequences. The b 20 diffusion scan uses the diffusion gradients to suppress signal from vessels between vessels and lesions.
The b 800 diffusion demonstrates the liver lesions. A coronal reformat is made from the b 800 diffusion scan. The window is inverted. This reformat nicely shows the liver with little distortion.

As this cases uses liver specific contrast agents, a delayed phase of the liver is acquired.
Again the resolution flexibility of mDIXON can be seen. In a 19-second breath hold a resolution of 1.2 mm x 1.2 mm in-plane resolution is acquired with a slice thickness of 1.75 mm.

Impact of using mDIXON and Ingenia 3.0T

Philips implementation of the mDIXON technique differs from other implementations. The “m” stands for “modified echo”, meaning that rather than being restricted to using an Out Phase and In Phase echo time, instead the shortest echo time is used. This allows a reduction in breath hold times, as well as more flexibility when selecting resolution.

The benefit of mDIXON is that it allows all echo types to be reconstructed in one breath hold whilst achieving a more robust fat suppression and shorter breath hold times, when compared to the eTHRIVE technique.
