ExamCard purpose

This ExamCard is applied in a comprehensive brain examination, with/without contrast. Includes DWI and DTI to examine possible stroke and visualize white matter fiber tracts.

ExamCard overview

3-P Brain
Scan 13-plane localizer
Scan 2Reference scan
Scan 3Ax T2W GRASE with CLEAR
Scan 4Ax T2W FLAIR with CLEAR
Scan 5Ax DTI 6 directions
Scan 6Sag 3D T1W TFE with SENSE
Scan 7Sag 3D T1W TFE post-contrast
Scan 8Ax E_sDW_SSh with SENSE

A three-plane survey is used. The GRASE scan visualizes bright fluid and/or blood. The FLAIR scan attenuates CSF signal so it is less likely to obfuscate the signal of nearby potential pathology.

The DTI is done to visualize white matter fiber tracts to facilitate avoidance of eloquent cortex in the event surgery is indicated

The sag 3D scans provides high resolution T1-weighted imaging and enables reformat into 3 planes. Contrast is injected prior to scan 7.

The DWI scan is done to ensure that vascular processes are not missed.

Patient preparation

Patient is positioned in the 8-channel SENSE Head coil. A forehead strap may be used to further immobilize head. If necessary, 5-10 mg i.v. sedation may be used.

Contrast agent / injection protocol

Following DTI acquisition, inject Gadolinium 0.1 mmol/kg.

Post-processing / analysis

Acquire ADC map on DWI and DTI acquisitions. Reconstruct sagittal 3D into axial and coronal planes.
