ExamCard purpose

This ExamCard is used for MRI of the liver/pancreas/biliary ducts in oncology context.
It is created on Ingenia 3.0T at release 4.1.3.

By positioning the patient with arms up, higher dS SENSE factors can be used in RL direction and the breath hold times are shorter. This can improve image quality.

Surv_BH is scanned to better align the following series with breath hold.
 
T2 FS is an axial T2-weighted fat suppressed scan with respiratory triggering.

AX T2 apnee is an axial T2-weighted breathhold scan.

mDIXON ALL is na FFE technique, scanned in breath hold mode for pre-contrast imaging and analysis of steatosis. It is also used to determine the coverage and breath-hold capacity of the patient before scanning with contrast agent.

mDIXON Dynamic (FFE technique) consists of a few series. 
  • DIX ART 20 is scanned 20 seconds after start of contrast administration. Used for the arterial phase.
  • DIX ART 50 is scanned 50 seconds after start of contrast administration. Used for the portal phase.
  • DIX P 80 is scanned 80 seconds after start of contrast administration. Also used for the portal phase.
  • DIX T 110 is scanned 110 seconds after start of contrast administration.

diff B1200 RL is an axial diffusion scan with 1 b-value and is used for the high sensitivity.


Optional scans:

3D MRCP (3D BILI) for pancreatic and biliary ducts visualization.

DIX Late Phase is an axial mDIXON series and can be used to scan a late phase.

Patient preparation

The patient is positioned supine and feet first. The patient is positioned with arms up to allow use of high dS SENSE factors in RL direction.

The dS Torso coil solution is used, with the Anterior coil in combination with the Posterior coil integrated in the table. The respiratory sensor is needed for the breath hold and respiratory triggered sequences.
 
An anti-peristaltic agent is administered to reduce motion artifacts due to bowel peristalsis.

For scanning the pancreas and biliary tree pineapple juice can be given to the patient prior to the examination.

Postprocessing

PicturePlus postprocessing is included in the ExamCard.
MIP and VRT can be done when needed.

Contrast agent / injection protocol

Gadolinium (Gd) based contrast agent is used.
