ExamCard purpose

This ExamCard is for shoulder examinations of large patients, whose shoulders do not fit in the standard shoulder coil. It uses CIA (Combined Image Aquisition) with Shoulder coil and Flex L+ coils.  R6.2 software and CIA license are needed.

 

 

ExamCard description:

This ExamCard contains protocols typically used in shoulder imaging. You can select the protocols required for your patient. Imaging times are about 5 minutes and SENSE is used in most scans.

 

All protocols can be use in any plane. However, remember to use horizontal phase encoding direction in coronal (L-R) and sagittal (A-P) scans, and vertical (A-P) in the transverse plane.

All protocols have over 200% phase foldover suppression, so you can just swap the phase.

 

This ExamCard has one scoutview, but you should rescan that as many times as needed to determine the right slice positioning. The position of the upper arm of the large patient is often compromised.

 

When imaging large patients with CIA coils, aliasing must be taken care of.

Shimming procedure for fatsat

When using the CIA method with a large patient and a large FOV, special attention for fatsat imaging is needed. This ExamCard has a special procedure to optimize fat suppression:

 

  1. Coil change for FatSat Shims
    Default coil is the Shoulder coil, but you can change that by double clicking the item and selecting "properties", if Flex L+ coil gives better signal.
  2. Shimming with one coil
  3. Coil change for FatSat scans
    Both coils are activated.

Positioning

Position the patient "Head left". Use "Low" table position to ensure the patient comfort.

 

Coil positioning:  Shoulder coil as high as it goes

                         Flex L+ coil around the collum

                         Flex L+ coil inside the Shoulder coil, then SENSE can be used in all directions
